Social Worker - Hospital Discharge Team

Liquid Personnel is recruiting Social Worker for its client's Hospital Discharge Team at Lancashire

What will your responsibilities be?

• Complete strengths-based assessments and facilitate safe, timely hospital discharges, ensuring individuals receive the appropriate support and services upon leaving hospital.

• Work collaboratively with multidisciplinary teams, healthcare professionals, service users, and families to develop and implement discharge plans that promote independence and reduce the risk of readmission.

• Manage a diverse caseload, carrying out Care Act assessments, reviews, safeguarding enquiries, and ongoing support planning in line with statutory requirements and best practice.

Qualifications & Experience:

To be successful in this role you must have, 

• Social Work England registration

• Eligible to work in the UK

• Hold a full UK driving licence

• Post-qualified experience

• Degree level or equivalent in Social Work
