Social Worker - Community Team

Liquid Personnel is recruiting Social Worker for its client's Community Team at Hull.

What will your responsibilities be?

• Completed Care Act 2014 assessments, reviews and support planning for adults with a range of care and support needs, ensuring person-centred outcomes were achieved.

• Undertook Mental Capacity Assessments and Best Interest decisions in line with the Mental Capacity Act 2005, promoting individuals' rights, choice and independence.

• Managed duty responsibilities and worked collaboratively with health professionals, families and partner agencies through multi-disciplinary meetings to coordinate effective support and safeguard vulnerable adults.

Qualifications & Experience:

To be successful in this role you must have, 

• Social Work England registration

• Eligible to work in the UK

• Hold a full UK driving licence

• Post-qualified experience

• Degree level or equivalent in Social Work
